For your convenience, we have provided the forms you will need for your first appointment.  Please print and fill out the forms below (Patient Registration Form & Patient Medical History Form) and bring them to your first visit.  All new patients are required to fill out both forms.  Mahalo.

The following form (Notice of Privacy Practices) is for your information.  Please review carefully, as you will need to acknowledge that you have read the forms on the Patient Registration Form (above).

The form below applies only to patients under the age of 18 who are not able to be accompanied by a parent/ legal guardian to their appointment.  The following consent form will need to be filled out prior to being seen by Dr. Sakamoto.
